Connecticut Health Requirements and Forms
Connecticut law states that no one may attend school without the medical documents identified below, unless written exemption is given on medical grounds.
The Health Assessment form (HAR-3) is mandatory for Pre-School, Kindergarten, Grade 6, Grade 9, and NCHS Athletics physicals and MUST have the doctor's signature AND stamp.
Required Physical Examinations:
New Students to the District:
ALL new students (including kindergarteners) must show proof of having received both a series of required immunizations (see below) and a recent (within 12 months) physical examination by a physician licensed to practice in the USA.
Preschool, Kindergarten, Grade 6 and Grade 9 Students:
ALL Preschool, Kindergarten, grade 6 and grade 9 students are required to provide proof of a physical examination completed during the school year. Immunization update required.
High School athletes participating in interscholastic sports: an annual physical exam is required.
Required Health Screenings:
Vision: Kindergarten, grades 1, 3, 4 and 5
Hearing: Kindergarten, grades 1, 3, 4 and 5
Scoliosis: Females in grades 5 and 7; Males in grade 9
